You may want to change your name for any number of reasons and that is completely up to you. However, if you want that name to be legally recognized, you'll need to go through the proper channels. We can help you with that.
What you'll need
- proof that you're 18 years or older (you need legal consent from your parents if you're younger than 18)
- acceptable identification
- proof that you are a resident of Alberta
- a completed name-change application form
- original birth certificates and identity documents or
- certified copies of immigration papers and a permanent resident card
- fingerprints
Although we can help you with the application, the actual name-change process is completed and approved or rejected by Service Alberta. Once the legal change of name has been completed, approved and registered with Service Alberta, you'll be issued an official name change certificate. If you need additional certificates, we can help you get those.
Did you know? If you are changing your name for an improper purpose, such as fraud or to avoid criminal charges or financial obligations, you will receive a fine that ranges from $500 to $2,000 or up to 90 days in jail.
